Clear education on discharge is key to prevent flare-ups and readmissions. QCH has dedicated a pharmacist who provides patient-specific medication discharge teaching — reviewing changes to home medicine regimens, resolving medication discrepancies that may lead to confusion at home, and making a follow-up call after the patient has been discharged.
